Complete the Referral form

We can tailor the service you require if you let us know more about your child by completing this form

After you first contact UP, you will be asked to completed a referral form. This allows us to collect some basic information that we use to make sure that we are the right service for you. Occasionally, we may decide that we cannot meet a child's needs in which case we will let you know the reasons why. Once you have completed the referral form we will contact you to clarify any additional requirements, make sure that we have understood the reason for referral, and have enough information to proceed with planning the assessment. At this meeting you will also be given a date for the face-to face assessments with your child. We will also send out questionnaires to be completed before the face to face assessments. These are for you as caregivers, schools and other adults that support your child if necessary. These questionnaires can be as important as the face-to-face assessments with your child.  

Every child's assessment needs are different, therefore we do not know the precise details of each assessment until after referral is processed.We conduct face-to-face assessments with your child. This will likely include a cognitive assessment and may also include measures of Learning Achievement, Executive Functioning or Memory. Your child will also be seen by a Speech and Language Therapist for an assessment of their language abilities. An element of all the face-to-face assessments will be clinical observation. If possible the assessments will be completed at your child's school. After all the assessment results are collated, an inter-professional discussion is held between the professionals within the clinic. This is the time when clinical interpretation is translated into actionable recommendations and outcomes. This truly multi-disciplinary approach allows health and educational professionals to work together to develop a holistic picture of your child's neurodevelopment and individualised, practical recommendations.

Report

A thorough report following the assessments which give you an insight into your child and guidance on how to move forward

Once we have collated all information from the questionnaires and face-to-face assessments we will write up the results in a report for you to share as you see fit. The report will have a summary of all the assessment results, observations, suggestions of likelihood of diagnoses and a set of recommendations specific to the child. Where necessary, there will be a section specifying how the neurodevelopmental results may impact on education and learning.The report is designed to be user friendly, but also thorough and comprehensive. For example reports click here.
